1. @classCode="SBADM" has been added as a required attribute.

2. I'm unable to fix this: it doesn't appear in the standard editor and temple fails to open this template.

3. Cardinality has been set to 0...1.

Description
1. In  hl7:entryRelationship   Dosage Instructions , the  hl7:substanceAdministration  is missing its mandatory @classCode="SBADM" attribute.

2. The  hl7:entryRelationship   Substitution act  contains the template  1.3.6.1.4.1.19376.1.9.1.3.9.2  but is annotated with  where  [hl7:act [hl7:templateId [@root='1.3.6.1.4.1.19376.1.9.1.3.9.1']]] , it should be .2 (as defined in the referenced template) . The Schematron is unable to validate this part (the single templateId is expected to be both .1 and .2 at the same time).

3. The  hl7:entryRelationship   Substitution act  cardinality should be 0…1 instead of 0…*, as per IHE PHARM DIS 6.3.4.5.3.19:
At most one substitution act element MAY be present to inform that a substitution occurred. When present, this element SHALL contain exactly one substitution event.
