According to the Kai's classification case #3 is closer to #1 rather than to #2.

IMHO, #2 is in any case an assertion (based on the information available) : e.g. the GP knows you have not allergies...(but it cannot be excluded...)

for cases #1 and #3 you cannot make any kind of assertion: the Clinician creating the IPS has no ideas if you have or if you have not allergies (there are no infos in your EHR about this topic); in that country data about allergies have not be collected, so the spider that generates the IPS cannot say anything about it...

I think that also for case #1 we should explicitly state that no info are available... (in case of the CDA template implementation with the appropriate nullflavor)

Here is my suggestion for the different use cases and their dataset representation:
  1. I do not know anything :: the List of Allergies/Intolerances is empty
  2. At the best of my knowledge you have no Allergies/Intolerances :: the List of Allergies/Intolerances contains a single item: No known Allergies/Intolerances
  3. I have no information about allergies and intolerance: what is the difference to 2?
  4. I know you had an adverse reaction, but I don't know the agent :: the List of Allergies/Intolerances contains an item Allergy/Intolerance with an agent of unknown (btw: this is rarely found in practice I think)
  5. I know you had an adverse reaction, but I don't know if this was actually the agent: somewhat similar to 4

how to collect in the data set the need for handling different cases as.

..If they are meaningful for the IPS scope...
