Description
CH-PHARM is the Building Block Repository for CDA-CH-EMED which defines the document exchange formats.

CH-PHARM consists of two parts:
  • Model of the IHE Pharmacy Technical Framework 2017 (PHARM; i.e. PRE, PADV, DIS, PML, MTP content profiles, IHE Pharmacy Content Profiles).
  • Swiss specializations of the IHE Pharmacy templates which can be used in other Swiss exchange formats and are used in CDA-CH-EMED

    Quentin, I think the definition in the Pharmaceutical Advice Item Entry Content Module are in error and cause the problem.
    The Organizer with the component there in Changed or Recommended Prescription Items is defined as an inclusion. With that the seperatableInd is in scope of the validation. It shall be a component with the Prescription Item Entry Content Module as a containment:

    Hope this helps.

    An observation: I see that you used inclusion very often which makes your templates often illegible. In most cases in the body, containment is the right thing to do.

    Because the data can be structured or represented by a text, information can be duplicated and there are no guidelines on how to reconcile that. The dosage instructions can appear three times in a CDA-CH-EMED document: as a structured data, in the non-structured entry and in the original representation.

    Propositions:

    1.     Templates Medication Treatment Plan Item Entry Content Module , Prescription Item Entry Content Module , Dispense Item Entry Content Module : when the dosage instructions are given in structured form, the entryRelationship element for the Dosage Instructions Non Structured Entry Content Module is forbidden. Else, the latter is mandatory.

    2.     If the structured data and the original representation (StrucDocText or PDF) differ, the source of truth is the former because the original representation can always be regenerated by a machine. A human could change the structured data to match the narrative if needed, by replacing the document or issuing a PADV CHANGE.

  • Issue 61 "Magistral preparations" with status "Open". Last event (2022-02-02T11:09:19):

    Magistral preparations are not so common but require much more data (and implementation work) than a regular drug. If we were not to support them, some part of the specifications could be ignored for now: the form code and the ingredients are not important when the GTIN is known, as drug databases usually contain them. The compendium can be used by patients too to get such information.

    Propositions:

    1.     Magistral preparations are not supported for now.

    2.     Template Manufactured Material Entry Content Module : the product code and name elements shall be filled and cannot contain a null flavor.

    3.     Template Manufactured Material Entry Content Module : the form code element is not supported, its description is updated.

    4.     Template Manufactured Material Entry Content Module : the ingredients are not supported, their description is updated.

    1.     These changes are temporary. Whether they are implemented in the aggregator only or reflected in the ArtDecor specifications is up for discussion.

    After internal discussion:
    This ValueSet is from FHIR Standard. We do not want to differ from the standard.  A recommendation is made in the FHIR IG: 
    For the representation of the time of administration according to the 1-1-1 scheme, the values MORN-NOON-EVE-NIGHT from the ValueSet EventTiming are highly recommended for the Swiss use.

    Therefore, this ValueSet will not be adjusted.
    A reduction would have to be discussed in a larger discussion.

    If this is good for you then I close the issue or set to "For future consideration"

    I'd like to propose to restrict substitution permissions to a single value, 0…1 instead of 0…*.

    Pros:
    • It prevents bad combinations, by example N + any other code that isn't currently disallowed.
    • It's easier to process for data consumers.
    • It's not harder, probably easier to choose for the data enterer.
    • Some of the combinations can be resolved to a single code.
    Cons:
    • Some of the combinations are not possible anymore.
    • N + any other code: it should be disallowed anyway.
    • (BC or G) + (TB or TG): it doesn't seem to make much sense. Is there an interest to only allow substitution for the same brand or for generics only?

    Yes, that is on purpose. That was the point what made the discussion so difficult: that v3 TimingEvent was not a genuine subset of FHIR EventTiming, but had this additional terms. The decision was to use FHIR VS in place of v3.

    Finding:

    In IHE PCC TF2 (revision 11.0), p.187, the dose (doseQuantity) and rate (rateQuantity) are defined by their low and high bounds that are different if a range is given, or equal otherwise (<doseQuantity><low value='' unit=''/><high value='' unit=''/></doseQuantity). 
    In IHE PHARM PRE (revision 1.9), pp.47-48, they are only defined by their value and unit (<doseQuantity value='' unit=''/>).
    In IHE PCC CDA (revision 2.6), p.141 and p.144, low+high is given in the definition and @value + @unit is shown in the example.

    The two standards disagree on the correct structure, Stéphane is investigating why PHARM is not following PCC here.

    In any case, the rateQuantity and doseQuantity IVL_PQ structure should be restrained to either @value + @unit or low + high. The examples that show a doseQuantity with a center are wrong for both PHARM and PCC.

    I guess the same correction should be applied to the template Dosage Instructions Dosage Change.

    The IHE PCC specification (§6.3.4.16.2) specifies the following element order: routeCode, doseQuantity, approachSiteCode and rateQuantity.
    The IHE PRE profile (§6.3.4.6.3) has a different (presumably wrong) element order: routeCode, approachSiteCode, doseQuantity and rateQuantity.
    Stéphane will open the discussion at IHE PHARM and presumably propose a CP to change the approachSiteCode element position and match the PCC ordering.
    I'll update the issue if/when it's voted.

    I've been unable to find the code U{PIECE(S)} anywhere.

    The code umol has an error in its display name, it should be "micromole" (the final "e" is missing).

    The second should be s, not S (Siemens).
    The minute should be min, not MIN.
    The hour should be h, not HR.
    The day should be d, not D.
    Parts per million should be [ppm], not PPM.

    U is not unit but enzyme unit (that could be very confusing). 10*3.U and 10*6.U are also wrong.

    I've not found the code kJ in any other value set or in the search service but it seems to be valid, k and J being defined as prefix and unit.
  • Issue 20 "TimingEvent VS differs from EventTiming in CH EMED" with status "Open". Last event (2021-02-18T17:46:15):
    Finding:

    At the projectathon 2020, the following problem popped up: The value set for timing event (such as before meal. After meal etc.) is not the same in FHIR and CDA-CH-EMED. But both value sets are required (by FHIR and IHE PCC, respectively)
