The Qualifier Observation template is a generic template that is referenced by various section or entry to enable the consistent communication of a qualifier observation. Qualifiers are free text or coded observations that qualify another element of the patient’s record. For example, in the Problem List a diagnosis may have Qualifier Observations attached to it that further qualify the diagnosis (e.g. laterality, disease stage etc.). The Qualifier Observation template supports coded or free text qualifiers, effective time, record identifiers and an author. |
